Information to have ready

• Requested date and pickup time.
• Pickup and destination locations.
• Appointment time or discharge situation, if applicable.
• Whether the rider is ambulatory or uses a mobility device.
• The type of wheelchair, walker, rollator or folding scooter, if applicable.
• Whether a return trip or waiting arrangement is needed.
• Whether the inquiry is for eligible Minnesota Medical Assistance transportation or private pay.

Advance reservations are encouraged. Same-day transportation may be available based on scheduling and vehicle availability. A request is not confirmed until we confirm the trip details and availability.
