Transportation information for care teams

Kingdom Concierge Services works with case managers, care coordinators, social workers, discharge planners, assisted-living teams, facility staff and families arranging non-emergency transportation. Contact us to discuss whether a rider, trip, schedule and mobility device fit our service.

• Capabilities: Ambulatory and wheelchair transportation; door-through-door assistance based on rider needs.
• Mobility devices: Manual and power wheelchairs, walkers, rollators and folding scooters, subject to fit and securement.
• Trip types: Medical appointments, therapy, recurring rides, hospital admission/discharge, round trips, pharmacy visits, private-pay personal outings and longer-distance Minnesota requests.
• Service area: Hennepin, Anoka, Ramsey, Dakota, Wright and Scott Counties; other Minnesota trips by request and availability.
• Hours: Monday to Friday 6:00 a.m. to 5:00 p.m.; Saturday 8:00 a.m. to 3:00 p.m.; Sunday closed.
• Payment: Eligible Minnesota Medical Assistance transportation and private pay.
• Credential: Minnesota STS Certificate of Compliance No. 383644.

How to begin a referral inquiry

Call 763-900-2812 or email Ride@kingdomconciergeservices.com with basic non-sensitive trip information. We will discuss the rider’s mobility needs, trip date and time, pickup and destination, return-trip needs, payment category and availability before confirming the ride. Please do not send diagnoses, Medical Assistance numbers or other sensitive health information by unsecured email.
